JOIN TEAM TRILOGY

The Infection Control/Wound/Medical Records Nurse (RN/LPN) is responsible for assisting the Director of Health Services and Assistant
Director of Health Services in planning, organizing, developing, and directing the Infection Control program, wound care program, and
medical records functions of the Nursing Department(s) in accordance with current federal, state, and local standards, guidelines, and
regulations that govern our health campus while ensuring that the highest degree of quality care is always maintained.

 

ROLE AND RESPONSIBILITIES


• Assists the Director of Health Services (DHS) and Assistant Director of Health Services (ADHS) to plan, develop, organize,
implement, evaluate, and direct the nursing service department, as well as its programs and activities, in accordance with current
rules, regulations, and guidelines that govern the Health Campus.
• Makes written and oral reports/recommendations to the ADHS/DHS as necessary/required, concerning the operation of the
nursing service department.
• Completes scanning and attaching of resident documentation in MatrixCare to ensure Medical Records are complete and
accurate.
• Assists in event investigations per ED/DHS/ADHS guidance.
• Oversight of the wound care program. Ensure wound measurements are obtained weekly and appropriate interventions are in
place. Monitor Braden Scale results to place preventative measures for high-risk residents. Participate in campus skin sweeps.
• Develops and maintains active Infection Control program ensuring binder and ATB stewardship.
• tracking is complete and reviewed monthly and as needed.
• Maintains accurate records of in-services and training in accordance with state regulations.
• Fosters team morale with recognition and appreciation of team members.
• Ensures orders are in medical record, care plans entered, CRCA sheets/profiles updated and other necessary elements including
binders/information for hospice and dialysis residents.
• Ensure resident vaccine information is entered into preventative health record.
• Ensure CareAssist documentation is completed.
• Lead as weight champion ensuring weight orders in place and weights obtained per order.
• Review and complete RD recommendations.
• Performs other administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc. as necessary.
• Participates in the development, revision, and implementation of resident care plans.
• Participates in survey and mock-survey campus preparedness.
• Participates in completion of survey plan of correction items/audits as necessary.
• Prepares, administers, and documents medications and treatments as ordered by the physician and/or included as nursing
interventions on the plan of care, as needed.
• Assesses residents for expected results of treatments and medications.
• Notifies supervisor, physician, resident’s legal representative of changes in condition, and takes appropriate action as required.
• Receives and documents physician orders according to facility-established standards of practice.
• Performs nursing procedures as required by resident assessment, physician orders and plan of care.
• Remains knowledgeable on use of equipment and teaches personnel how to operate equipment when applicable.
• Provides direct nursing care as necessary.
• Maintains first impressions to welcome new and potential customers to experience our services.
• Ensures campus is tour ready including back up team resources, tour path, model room, first impressions, and tour stations.
Executes tours as necessary.
• Makes daily rounds of the nursing service department(s) to ensure that all nursing service personnel are performing their work
assignments in accordance with acceptable nursing standards. Complete review of fridge temps, FirstDose room temps BID,
completion of crash cart audits, Infection Control rounding, fall intervention check, med carts, HIPPA violations, TSS, med
rooms. Reports findings to the ADHS/DHS and the Executive Director.

WHAT WE'RE LOOKING FOR

• Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel,
vendors, and the general public.
• Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that
pertain to long-term care.
• Must possess, at a minimum, a Nursing Degree from an accredited college or university.
• Must have and maintain a current, valid state RN/LPN license.
• Current, valid CPR certification required.
• Infection Preventionist certification
